The New Albany-Floyd County Consolidated School Corporation is coordinating the collection of selected information items on children with disabilities, birth through twenty-one years of age, for the purpose of determining present and future program placement needs, and for statistical reporting. Information will be sought from other agencies serving disabled children as well as the general public.
Information which could identify an individual child will not be maintained beyond the local level collecting it and will not be held by any State level agency unless parental consent is obtained. Data items to be collected include professional diagnostic information services needed and provided.
Parents and guardians maintain the right to inspect any and all data which is subject to collection and to appeal the accuracy of any such data. The access of unauthorized persons to personally identifiable data without parental consent is forbidden.
